We want to generate a montly report from our billing system that will tell
us how many sales -- from all sources -- broken down by product and revenue
amount.

The purpose of this report is to know how we, as a company, did during a
particular month and also be able to easily compare months, etc.

I guess you could call this a combination marketing, sales, and revenue
report?

Any suggestions on what exactly what information should be in the report?
How it should be formatted?

Any websites where I could download a template for this or could someone
post a template here?

I think the first step is deciding exactly which fields need to be on the
report. Once you have all the field names it should be easy enough to layout
a sample report using Excel (even if you use fake numbers). From there, its
just a matter of having a programmer link to the data in your database and
generate the physical report.

Your accountant may have some built in reports that can be pulled from
whatever software program it is that she uses to keep your books.
